Program Overview

  • Patients monitored by home care can reduce hospital admissions
  • Program promotes self management
  • Provide physician with detailed reports of Pulse Oximetry Study

Goals

  • Nurse initiated overnight Pulse Oximetry
  • An individualized Pulse-Oximetry report sent to physician within 24 hours
  • Physician can recommend o2 therapy and home care, based on report and home bound patient status
  • Overnight Pulse-Oximetry after o2 recommendation assessment
  • RN follows individualized education Re: Disease process, weight management, nutritional counseling, sodium restrictions & o2 precautions
  • RN will assess/evaluate cardiac status, and provide lab draws to monitor electrolytes as ordered
