Program Overview
Our certified wound ostomy continence nurses will assess wound care needs. The nurse works with the physician to develop and implement a plan of care designed to educate the patient/family to achieve maximum independence.
Wound Care Services
- Dressing recommendations on all pressure ulcers, arterial/venous ulcers, surgical wounds, diabetic foot ulcers, or others
- Patient and family education
- Sleeping surface/wheelchair pressure reduction with relief recommendations
- Conservative sharp wound debridement pending physician's orders
- Pressure ulcer prevention measures for high risk patients

Program Overview
To assist the cardiac patient during recovery following a cardiac event.
Patients Who May Qualify
- Open heart surgery
- Angioplasty or cardiac catheterization
- Cardiac event
- Pacemaker or defibrillator
- Cardiac arrhythmia
Additional Skilled Services Provided
- Venipuncture for lab work
- Observation monitoring and teaching to patient and/or caregiver
- EKG monitoring and pulse oximetry as ordered in home
Highly Qualified & Licensed Staff
Program Overview
- Maximize recovery post-op laminectomy and spinal fusion (spinal stabilization concept)
- Pain management and patients' participation in symptom control
- Correct body mechanics - all aspects of patient's activities including : positioning, transfers, ambulation, ADL's (including grooming & hygiene)
- Appropriate progression of therapeutic exercise/ambulation/activity according to patient's tolerance: emphasis on neutral trunk position to allow for optimal postural gains
- Diabetic Educator recommends change in patient's diet, exercise and lifestyle
- Instruction on the use of syringe for self administration of insulin
- Promote safe administration of oral medications
- Education of proper disposal of contaminated supplies
- Education on use of glucometer and its results
- Provide physician with detailed pulse oximetry report
- Physican can recommend oxygen therapy
- Educate patient regarding disease process, weight management and diet restriciton
- Assess cardiac status and provide lab draws to monitor electrolytes as ordered
- Understanding risk factors and complications of a stroke
- Working with speech pathologist, occupational therapist, and physical therapist to improve the quality of life
- Education about blood pressure and anticoagulant medications
- Assisting caregiver and patient in coping with lifestyle changes

Total Joint Replacement, Post Arthroscopy, Musculo-skeletal Injury, Physically Deconditioned Patient
- Appropriate progression of therapeutic exercise, ambulation and activity
- Pain management, maximize healing process
- Promote a safe level of functioning for patient and caregiver
